Summer Camp 2023

Medical Forms

It's time to update med forms for 2023. Med forms are only good for one year. Summer Camp will not take the previous version this year. Forms A, B1 and B2 are required for all adults, siblings and scouts that attend any activity. These can be completed by the parent.

  Also, T41 needs your vaccination record and copy of your insurance card. Med Form C will be required if you are attending summer camp. Form C must be completed by a physician. 

When turning in med forms, please bring 3 copies to a scout meeting, or send them by US mail to Mrs. Fox. Do NOT email these forms.
