Submit new medical forms (you have not done so since July 2025)

Summer Camp will be held at Camp Crooked Creek every year
Medical Forms
Med forms are only good for one year. Summer Camp will not take the previous version this year. Forms A, B1, B2, and C are required for all adults, and scouts that attend summer camp.ย Forms A, B1, and B2 are to be completed by the parent, and form C must be completed by a physician.ย
In addition to the forms, a copy of the vaccination record and medical insurance card is necessary.ย
When turning in med forms, please bring 3 copies to a scout meeting, or send them by US mail to Mrs. Fox. Do NOT email these forms.

Total cost of summer camp is $TBD.ย The Troop will be using all proceeds from our fall popcorn sales to subsidize this cost.ย ย
Deposit of $100 is due by February 24th
Second installment of $150 is due by March 24th.
